It is Just 4 Kids Preschool’s policy to allow no less than two (2) weeks before we can assign a start date to a new enrollee. You are asked to fill out this waiver because you requested for an earlier start date that is earlier than our “No less than Two (2) weeks start date” policy.

For us to serve you better, we need to know if you qualify for an earlier start date by filling out this form. Depending on your answers, you will be asked to fill out subsequent form(s) that is/are due to be submitted on or before your requested start date, together with the other Community Care Licensing required documents. We reserve the right to deny an earlier start date if you fail to submit the subsequent form(s).
