Pesticide Schedule Acknowledgement Form
The State of California requires all childcare facilities to notify all staff and parents whenever our center is treated with any type of pesticides. This acknowledgement form is to inform you of the pesticides we will be using during the year. It is your responsibility as the parent or legal guardian to notify us if your child has certain asthmatic reactions to any of the pesticides we will be using.
We have contracted ORKIN, a refutable pest control company, to spray for ants & insects four times per year. This treatment will be scheduled quarterly. We will post warning signs no less than 2 days before we spray the facility and the signs shall remain posted for 3 days after our center has been treated.
ORKIN has provided us with the names of the pesticides they use at our facility. Please make sure that your child is not allergic to any of these listed products:
- Talstar Granular by FMG
- Phantom by Bask.
